Referral Requests

In order to have your insurance pay for your visit to a specialist or other health care provider, it is necessary for us to process a referral form. We will need to do this for each appointment.

The phone number for referral requests is 412-247-9718.

When you call the referral line provide the following information:

• Patient Name
• Patient Phone Number
• Insurance Company Name
• Name of the Physician you will be seeing
• Physician Phone Number
• Physician Fax Number
• Date of the scheduled appointment
• Reason for the Referral

Notify our office 5 days in advance of your scheduled appointment.  This gives us time to process all of the referral paperwork. It is the patient's responsibility to request the referral in a timely manner.

Same day referrals will be done only in the case of an emergency.

Referrals will not be completed after you have been seen by the specialist.
